I am a recent convert to Dragon Systems Point and Speak program. This is a very good program for inputting text into any Windows app. I have not yet tried it under any other operating system.

Right out of the box one need only click the mouse where you wish to dictate. Whether there is a macro for filling in forms or whether there is a form design program which would work with Point and Speak I do not know. Dragon Systems has pioneered his field since the beginning. With Point and Speak my opinion is that voice recognition is ready for the mainstream. I think the company has reached critical mass and will be marketing in many specifically tailored areas such as the one you describe. If not, they are licensing to developers who will!
